docs(readme): move shipped agent-chat + webhooks out of Roadmap#60

These two capabilities shipped in v0.1.0 (v2 tool layer) and are already listed under Tools, but the Roadmap still presented them as upcoming — a direct contradiction. Replace the two bullets with a 'Recently shipped' note linking to the Tools section.

What
The README Roadmap section still listed Agent Chat via MCP and Webhook Triggers as planned — but both shipped in v0.1.0 and are already documented under Tools → Agent Chat & Webhooks. That's a direct contradiction for anyone reading top-to-bottom.
